Step 3: Key In “Existing Policy No.“
Step 4: Select “ENHANCE/UPGRADE/CONVERT EXISTING MEDICAL RIDER OR HEALTH PLAN“
Step 5: Done
Part 2: DECLARATION OF COMFIRMATION BY PROPOSER
Tick: Second Box
“I confirm that i am aware of the effects of replacement of policies / rider and wish to upgrade my existing coverage through the purchase of this new Policy”
