You must apply within First Year after the policy inforce.
Otherwise, you cannot apply again in the future.

Before apply must go Great ID or MPOS acknowledge policy first.
All application done in Great ID only

Statutory Declaration (for Loss of Policy & Policy not received by the Policy Owner)  PSF08

Due to company will update form version from times to times,
kindly download latest version direct from Gesica or Epartner.

1. Policyholder / agent needs to fill up PSF08 (Statutory Declaration Form) and make payment of RM40 :

– RM 20 processing fee
– RM 10 revenue stamp for Statutory Declaration Form
– RM 10 stamp duty paid to validate the policy contract

2. The completed PSF08 form also needs to be signed / declared before Commissioner for Oaths or other officer empowered by Law to administer oaths.

3. Once all the required documents are received, the request will be processed within
10 working days. The duplicate policy contract will then be posted to Policyholder’s mailing address via registered mail.
